Rupatadine is a long-acting, non-sedating H1 receptor antagonist with an additional ability to block platelet activating factor (PAF). Since both histamine and PAF contribute to bronchoconstriction, vascular permeability, and inflammation, this dual mechanism provides enhanced therapeutic benefit compared to antihistamines alone. Rupatadine also prevents mast cell degranulation and reduces the release of inflammatory cytokines such as TNF-α, contributing to its strong anti-allergic action.
Common: Asthenia, dizziness, drowsiness.
Uncommon: GI upset, constipation/diarrhea, dry throat, nausea, rash, weight gain, irritability.
Rare: Palpitations, tachycardia.
Adults & adolescents (12+ years): 10 mg once daily, with or without food.
Children (2–11 years):
≥25 kg: 5 ml oral solution once daily.
10–<25 kg: 2.5 ml oral solution once daily.

With medicines: Concomitant use with ketoconazole, erythromycin, or other CYP3A4 inhibitors may increase systemic exposure. Caution with statins, CNS depressants, or alcohol.
With food: Avoid grapefruit and grapefruit juice during treatment.
